More than one treatment
Many of the treatments presented on our website require more than one treatment, and each person responds differently to individual treatments.
Dr. Steven Swengel and the staff of RefinedMD cannot make any guarantees about the results after only one treatment.
Price Quotes
Price quotes are only guaranteed for 30 days. Cosmetic Procedures are not Medically Necessary. All cosmetic procedures are not considered medically necessary. Therefore, the charges will not be billed to personal insurance companies and are completely the patient’s responsibility.
Payment for multiple cosmetic treatments or a treatment package is due in full at the time of scheduling. No shows or cancels less than 24 hours prior to treatments will be charged a portion of their pre-payment.
Specials and Promotional Pricing
All of our specials and promotions are subject to individual limitations that are at the discretion of the practice. For some specials, a minimum purchase is required. Some specials may be restricted to new patients only. Some specials must be redeemed in a single visit. Specials and promotional pricing offers cannot be combined. Promotions can only be redeemed once per patient. We make an effort to be upfront and publish our terms and conditions at the time of the offering. Please contact RefinedMD for specific terms and restrictions applicable to the special you are interested in. For neurotoxin specials a minimum spend of 20 units is required to redeem the special.

Deposits and Cancellations
For individual cosmetic procedures, a deposit is required to secure your appointment. The amount varies based on the treatment. If patients need to cancel please give 24-hour notice. This deposit is non-refundable if the appointment is not kept, or canceled within 24 hours of treatment.
Patient financial responsibility:
A patient is responsible for any and all charges for services rendered and not paid by insurance. This includes medical service, lab or diagnostic testing and any other service performed by the physician or physician’s staff.
Patient understands that it is their responsibility to know if their insurance has a Deductible, Co-payment, Co-Insurance, Out of Network, Unusual and Customary Limit or any other type of benefits limitation for the services she/he receives and she/he agrees to make full payment.
Patient understands that it is their responsibility to know if the medical care professional and or physician they are seeing is a contracted in-network provider recognized by their insurance company or plan.
If the physician they are seeing is not, it may result in claims being denied or higher out of pocket expense to she/he. Patient understands this and agrees to be financially responsible and make full payment.
Patient understands that in the event that their insurance is not valid or coverage has been terminated at the time the services are rendered, they are responsible for the full amount of their visit and or any procedures rendered.
Patient understands that should their account become delinquent; they shall pay the reasonable attorney fees or collection expenses, if any.
For patient convenience, practice accepts Visa, Mastercard, American Express, Cash, and Personal Checks.

THE FOREGOING LIMITATIONS, EXCLUSIONS, AND DISCLAIMERS SHALL APPLY TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, EVEN IF ANY REMEDY FAILS ITS ESSENTIAL PURPOSE.
