RefinedMD is located in Los Gatos and serves all of the greater San Jose area. Led by Dr. Steven Swengel, board certified dermatologist, we invite you to learn more about how you can get fantastic skin with our variety of medical and cosmetic treatments available at our practice. Of course, we accept medical insurance, but we also offer a variety of aesthetic services that complement your treatments. Sometimes these are covered by insurance as they are medically necessary, other times they fall into that pay out of pocket area. Either way, our staff will walk you through your treatment options so that you can make the best decision for you, your skin, and your wallet.

A Note About our Insurance Policy Moving Forward

As of June 2021 RefinedMD is in the process of terminating our medical insurance contracts with all commercial insurance companies, including medicare. We will post effective termination dates here which represent the last date we will be in network with the listed carriers.
We will continue to do our best to optimize availability for existing patients who need medical/dermatologic appointments. Availability for appointments for medical visits for new patients may be beyond 4 weeks at this time

We look forward to continuing to provide your dermatologic care, however charges will be due at the time service is rendered. We will provide you with a medical billing statement that you can remit to your insurance and make a claim for out of network benefits, if you have that coverage.

Questions and Answers

What does Out-of-network provider mean? Out-of-network means that a doctor or physician does not have a contract with your health insurance plan. Your benefits and coverage may be reduced. Some health plans, such as an HMO plan, will not cover care from out-of-network providers at all, except in an emergency.

Why are we transitioning to Out-of-network providers? Our staff of Board Certified Dermatologist and Facial Plastic Surgeons have reached the conclusion that it is in the best interest of the practice to withdraw from our contracts with health insurance plans. 

How do I know if I have Out-of-network benefits? You can call the toll free member services number on the back of your card for more information about your plan coverage. 

When will we become out-of-network providers? This date varies by plan. We recommend that you call your member services department in order to verify accuracy with your specific health insurance plan. 

Who will file the insurance claim? As a courtesy to you, we will file an out-of-network claim for you to your health insurance plan. If you have an appointment scheduled after this date please contact our office and let us know how you would like to proceed. 

Who is responsible for payment? You will be responsible for making payment in full at the time of service. If your health insurance plan makes payment, the payment will be mailed directly to you. 

RefinedMD will initiate billing with your insurance carrier here at the office. However, this does not assure payment by your insurance carrier. Patients are responsible for all unmet deductibles, applicable co-payments, and coinsurances. If we are not in network with your insurance, and you still choose to see one of our providers, you will be responsible for all office fees at the time of your visit. Any and all outstanding patient balances are due upon receipt or next visit.

We will, of course, wish to see you even if your insurance is out of network and you’ll simply pay for the cost at the time of service. Please call the number on the back of your card to verify coverage.

Cosmetic and Aesthetic Services

Cosmetic Treatments at RefinedMD

RefinedMD accepts health insurance for a variety of procedures and additional cosmetic treatments that, while they produce fantastic results, the treatments are generally not covered by insurance.

When opting for cosmetic treatments such as Botox, Dermal Fillers, Microneedling and more, payment is due for all cosmetic procedure fees at the time of your visit. This includes procedures deemed cosmetic by the provider, including but not limited to, removal of skin tags, removal of benign moles, and removal of keratosis or seborrheic keratosis. All laser procedures are considered cosmetic unless the provider specifies otherwise. If you subsequently bill your insurance for a cosmetic procedure and your insurance erroneously covers the procedure, you are not entitled to reimbursement by our office and we are obligated to notify your insurance carrier of their error.

Payment for all non-medically necessary services, self-pay medical services, and insurance co-payment, is due at the time of your visit. We accept American Express, Visa, Master Card, Personal Checks, Cash, Alphaeon Credit, and Care Credit. Refined Gift Cards are also available.
