Great skin is possible and we look forward to a proper evaluation from which we can build recommendations for your particular skin. Here are a few things you’ll want to know, including access to our patient portal, office policies, appointment information, and more.

RefinedMD Patient Portal

RefinedMD offers a patient portal to all patients when they schedule their appointment. Easy access to our portal is essential as you’ll find patient forms, copies of labs, editing appointment times, communication with our office, and more. If you do not know your information to log in, please call or email us.

Office Policies

1. Office Philosophy

At RefinedMD, we pride ourselves on providing all of our patients with professional and compassionate care. We strive to be on time for each and every appointment. We answer the phone in person, during regular business hours. We offer easy parking. Learn more about our office location and hours of operation.

2. Appointments

To schedule an appointment, please call (408) 688-2082. We will work closely with you to accommodate your scheduling needs.

 

New patients are always welcome. Within three days of your scheduled appointment, you will receive an email providing you with a link to our secure patient portal. There, you can complete all New Patient forms ahead of time. If for any reason you are unable to access the patient portal, you can print out the New Patient forms by clicking on Medical Intake Form. After completing the forms you may bring them with you to your appointment or email them to us at info@RefinedMD.com.

3. Also, please remember to bring:
  • Your insurance card(s) both primary, as well as, secondary if applicable. We will need the name and birthdate of the primary insured for all insurances if different from the patient.
  • A list of all medications you are currently taking, both prescribed and over the counter (OTC).
  • All dermatologic medications you are currently using for your skin.
  • Saturday Appointments
  • Saturday appointments require a valid credit card to hold appointment time. Failure to keep your appointment will result in a $100 fee unless properly notified.
4. Cancellations & Missed Appointments

We understand that life happens and sometimes you need to reschedule your appointment. We ask that you please give us at least 24-hour notice. In the event you do not make your appointment and fail to notify us in a timely manner, all subsequent visits will require a $100 deposit. Additional “no shows” or late cancellations will result in the forfeiture of the $100 deposit.

 

If you find yourself running late, please call our office. We will either let you know that we can still accommodate you or we will reschedule your appointment for another time. Late arrivals may impact the length of your treatment. Your service will end on time so that the next patient is not delayed.

 

Appointment reminders are made 72 to 48 hours prior to scheduled appointments via text message and/or telephone.

In the rare instance you have a dermatologic emergency after business hours please call (408) 688-2082.

5. Financing and Payments

6. Cosmetic Visits

Payment is due for all cosmetic procedure fees at the time of your visit. This includes procedures deemed cosmetic by the provider, including but not limited to, removal of skin tags, removal of benign moles, and removal of keratosis or seborrheic keratosis. All laser procedures are considered cosmetic unless the provider specifies otherwise. If you subsequently bill your insurance for a cosmetic procedure and your insurance erroneously covers the procedure, you are not entitled to reimbursement by our office and we are obligated to notify your insurance carrier of their error.

7. Cosmetic Packages and/or Series

When purchasing a series of treatments, services are provided at a discounted price. Therefore, payment is to be made prior to starting a series.

8. Retail Product Returns

Unopened and unused products are returnable within three days after purchase with a receipt. Open products may be returned within three days with clinically documented adverse reaction only. Prescription products (i.e. Latisse, Hydroquinone, Tretinoin, etc) are prohibited by law to be returned. If there is an adverse reaction, please call our office as soon as possible or within three days post-purchase.
